What you get back is not an assessment. It is your own account, organised, with the questions your particular gaps make worth asking and a clear line between what you reported, what a document is said to say, and what only somebody on site can settle.

How this record works
- It never submits anything. Creating, editing, printing, or reading the record sends nothing. The only way any of it reaches a provider is the explicit choice on the request form, and even then a narrow summary goes rather than the record.
- It is saved in this browser only. No account, no backup, no sync to another device. Clear your browser data and it is gone. There is a delete control at the bottom of the workflow.
- Unknowns are answers. Up to 4 tests can be recorded, and every field can be left as “not stated”. A record that says “unknown” honestly in nine places is more useful in a conversation than one filled with guesses.
- Nothing you type is measured against a score. There is no readiness score, no risk meter, and no completion percentage — only a count of what you have answered.

Your record can travel with a request if you choose to attach it — you will see exactly what would be shared, and you can decline and keep the record.
